How different is Catalan from Spanish?

Both are Romance languages derived from Latin. 

The Romance languages (less commonly called Latin languages, or Neo-Latin languages) are the modern languages that evolved from "Vulgar Latin" between the third and eighth centuries. 

Everyone in Catalunya region of Spain speaks "castellano" which is Castilian Spanish. This refers to the Spanish language as a whole. Locals are happy if you speak a few words of Catalan but for tourists it's fine to use Spanish too.
